MyChart is a free, secure online tool providing patients and designated family members and partners in care at Northumberland Hills Hospital access to medical records and personal health information. With MyChart, you can view your test results and other health information from seven partner hospitals:

MyChart is a free and secure online tool providing you and your designated family members, friends and members of your health care team access to your personal health information.

MyChart information includes:

  • Lab results
  • Diagnostic Imaging reports
  • Discharge summaries and clinical notes on your care or treatment

You also have the ability to upload other health information to customize the portal as a personal health record.

MyChart provides you with seamless access to your health information, and encourages you and your family to be engaged throughout your care journey. Having this information can help you manage your care with your health care team and can lead to better collaboration and communication.

All NHH patients will be able to register for MyChart, however, only patients who have completed the registration process will have access to personal health information online through MyChart. Registered patients can electronically grant access to family members, friends, and members of their healthcare team to their MyChart portal.

MyChart is a free tool that provides you with easy and timely access to your health information online, at any time of day. You can use MyChart to create, manage and enter your own personal health information, such as your mood, blood pressure, medications, immunizations, allergies and your height and weight.

MyChart provides you with seamless access to your health information, and having this information available can help you manage your care with your healthcare team and lead to better collaboration and communication between you and your healthcare providers.

Beginning December 3, 2021, NHH patients can sign up for the new MyChart Central East by:

The online portal is accessible from anywhere at any time through the Internet on a computer, laptop, tablet or mobile device.

You can download the MyChart app for your Apple or Android device to manage your health information on the go. To install the MyChart app, go to the App Store or Google Play Store and search for “MyChart.” The mobile apps contain many of the same features as the MyChart website. Please ensure the hospital where you receive care is selected before signing in. If you selected the wrong organization, click “Switch organization” above the log in details.

You cannot access MyChart without a valid e-mail account. You can sign up for a free email account using a variety of services (e.g. Gmail, Yahoo, iCloud, etc.).

Yes, you can follow our current process of requesting your personal health information record through the Health Records department.

MyChart is free for patients.

Sunnybrook Health Sciences Centre developed MyChart™ 12 years ago and it has since been adopted in a number of other hospitals across Ontario.

The following partner hospitals are all launching MyChart (MyChart Central East) in December 2021:

  • Campbellford Memorial Hospital
  • Haliburton Highland Health Services
  • Lakeridge Health
  • Northumberland Hills Hospital
  • Peterborough Regional Health Centre
  • Ross Memorial Hospital
  • Scarborough Health Network.

There is currently no link between PocketHealth and MyChart.

PocketHealth is a secure, online storage hub for medical imaging records. When you visit an imaging centre linked to PocketHealth, such as NHH’s Diagnostic Imaging department, you can have access to a permanent, digital copy of your medical imaging record that can be shared with any healthcare professional. To enroll, patients can visit www.pocket.health/NHH. Once enrolled, patients can view their available medical imaging history from NHH, sharing imaging instantly with any physician, or burn their own CD/USB from home. There is a fee at the time of enrolment which includes permanent access to all previous imaging and radiologist reports as well as any new exams administered over the following two weeks. Patients can update their accounts with new NHH records any time for an additional fee.

MyChart is secured through the same encryption technologies used by the major banks for online banking.

If you lose or forget your password, you can reset it by clicking the “forgot password” link on the MyChart login page.

Your doctor or nurse will discuss your test results with you at your next appointment.
