LOCATION & ABOUT NHH
Located approximately 100 kilometres east of Toronto, NHH delivers a broad range of acute, post-acute, outpatient and diagnostic services. Acute services include emergency and intensive care, medical/surgical care, obstetrical care and palliative care. Post-acute specialty services (PASS) include restorative care and rehabilitation. Mental health care, cancer and supportive care, dialysis and other ambulatory care clinics are offered on an outpatient basis through partnerships with regional centres and nearby specialists. NHH offers a full range of diagnostic services, including magnetic resonance imaging (MRI), computed tomography (CT) and mammography. SUMMARY OF DUTIES
Reporting to the Patient Care Manager, as a member of the multi-disciplinary team, the PSW provides quality patient centered care to Post-Acute Care patients including, but not limited to, assisting patients with their personal hygiene needs, activities of daily living and mobility according to their plan of care. The PSW will support exceptional mouth care and feeding, as well as skin care for the treatment and prevention of Hospital Acquired Pressure Injuries. The NHH PSW will assist our patients living with dementia, or behaviors by implementing stimulating activities, sitting, and talking with patients, or engaging in patient interactions that match their care plan. The NHH PSW will work and document autonomously under the supervision of a regulated health care provider.
QUALIFICATIONS REQUIRED

Personal Support Work - Ontario College Certificate from an accredited community college

Basic Cardiac Life Support (B.C.L.S.) program, Level II: Basic Rescuer certification

Current CPR & First Aid
-
Registration with the Health and Supportive Care Providers Oversight Authority
-
Knowledge and experience using proper body mechanics for a one-person transport by wheelchair or stretcher; demonstrated understanding of proper body mechanics when pushing, pulling, lifting and transferring patients; working knowledge of stretchers, beds, wheelchairs, etc.

Knowledge and experience using proper body mechanics for a one to two-person transfers to assist with routine mobilization of patient at a variety of functional levels
-
Knowledge and experience with mobility devices, including wheelchairs, walkers, lifts, etc.

Excellent verbal communication skills and the ability to work effectively with patients, families, and other members of the healthcare team
-
Applies NHH values and professional practice standards to patient/client care and service.

Basic computer skills word, excel, power point etc.

Knowledge and understanding of person and family centered care with a geriatric focus

Recent patient care experience as a PSW in an acute care, long term care or rehabilitative setting preferred

Gentle Persuasive Approach Certified Preferred (GPA)

EPIC experience an asset

May be required to wear personal protective apparel (e.g., Gowns, gloves, masks, goggles, respirator etc.) while performing duties; may at times be subjected to unpleasant tasks, sights and odours

Follows safe and healthy work practices and maintains a safe and healthy work environment through attendance at educational programs, use of established reporting mechanisms for injuries, illness and hazards and adherence to health and safety related policies and procedures

EMPLOYMENT REQUIREMENTS
Successful applicants are required provide a satisfactory vulnerable sector criminal reference check, that was complete less than one year from the date of hire. This requirement must be met within the first sixty (60) days of employment.
Additionally, to the above, successful applicants are also required to complete the Health Care Worker Health review process which includes providing an up-to-date immunization record to ensure compliance with the Ontario Hospital Association Communicable Diseases Surveillance Protocols within the first thirty (30) days of employment. Northumberland Hills Hospital operates a mandatory COVID-19 vaccination policy for all staff.
