LOCATION & ABOUT NHH

Located approximately 100 kilometres east of Toronto, NHH delivers a broad range of acute, post-acute, outpatient and diagnostic services. Acute services include emergency and intensive care, medical/surgical care, obstetrical care and palliative care. Post-acute specialty services (PASS) include restorative care and rehabilitation. Mental health care, cancer and supportive care, dialysis and other ambulatory care clinics are offered on an outpatient basis through partnerships with regional centres and nearby specialists. NHH offers a full range of diagnostic services, including magnetic resonance imaging (MRI), computed tomography (CT) and mammography. The hospital serves the catchment area of west Northumberland County. ROLE OVERVIEW

Reporting to the Director of Clinical Services, the Pharmacist is responsible for the clinical and distributive functions related to the practice of pharmacy throughout Northumberland Hills Hospital (NHH). It will be expected that the most qualified pharmacist provides clinical services to most areas of the organization, including the acute care, emergency, and post-acute care units.

Primary responsibilities:

  • Provide clinical services, including review and monitoring of drug therapy
  • Ensure that all medications are distributed and administered in a safe and timely manner
  • Act as a resource to the members of the interdisciplinary team, including the provision of drug information, answering to pharmacy consults and attendance at patient care rounds, as required
  • Assist in the reporting, investigation and follow up of adverse drug events and medication errors
  • Provides education to the interdisciplinary team members and patients, as required
  • Assist in ensuring that the acts and regulations related to pharmacy practice are adhered to and recommend changes in the pharmacy practice area as appropriate
  • Maintain records as required by Northumberland Hills Hospital, the Food and Drugs Act, The Ontario College of Pharmacists and the Controlled Drugs and Substances Act
  • Provision of on-call support to interdisciplinary team, as required
  • Monitor quality measures, as assigned
  • Assist with the development of policies related to drug utilization, distribution and control

QUALIFICATIONS/REQUIREMENTS

  • Current registration in Part A of the Ontario College of Pharmacists
  • B.Sc Phm required; PharmD and/or hospital pharmacy residency preferred
  • Demonstration of strong clinical, prioritization, communication and interpersonal skills
  • Preference will be given to candidates with at least 2-3 years of hospital pharmacy experience

EMPLOYMENT REQUIREMENTS

Successful applicants are required provide a satisfactory vulnerable sector criminal reference check, that was complete less than one year from the date of hire. This requirement must be met within the first sixty (60) days of employment.

Additionally, to the above, successful applicants are also required to complete the Health Care Worker Health review process which includes providing an up-to-date immunization record to ensure compliance with the Ontario Hospital Association Communicable Diseases Surveillance Protocols within the first thirty (30) days of employment. Northumberland Hills Hospital operates a mandatory COVID-19 vaccination policy for all staff.
