LOCATION

Located approximately 100 kilometres east of Toronto, NHH is a vibrant community hospital delivering a broad range of acute, post-acute, outpatient and diagnostic services. Directly serving a growing population of more than 60,000 residents spread across a mix of urban and rural settings, NHH directly employs approximately 750 people and relies on the additional support provided by a highly engaged and supportive network of physicians, midwives and volunteers.

ROLE OVERVIEW

Reporting to the Clinical Manager, Access and Flow and Allied Health the BSO nurse will provide support and expertise to guide practice and care planning for patients who are experiencing responsive behaviours related to dementia, mental health, addictions, or other neurodegenerative conditions at Northumberland Hills Hospital (NHH). The BSO RN will play an integral role in collaborating, providing expertise, and will support interdisciplinary teams across the organization, including Home and Community Care Support Services (HCCSS), and other practitioners involved in the circle of care.  This role will build, facilitate, and foster collaborative cross-sectoral relationships, build capacity within the teams to support patients living with responsive behaviours, and facilitate seamless transitions in care.  The BSO RN will utilize their leadership skills and expertise in developing competencies in the hospital to enable others to develop knowledge and confidence in their ability to support patients and their care partners. The role will also assist in facilitation of care coordination for complex patients.

QUALIFICATIONS/REQUIREMENTS

  • Registration in good standing with the College of Nurses of Ontario
  • Completed PIECES Learning and Development Program required
  • Gentle Persuasive Approach (GPA) training required
  • Completed Montessori Dementia Certificate Program required
  • Canadian Nurses Association Gerontology Certification GNC(C) required, or completion within 1 year
  • Knowledge and experience of best practices working with dementia, delirium, and other neurological patient conditions
  • Minimum of three years’ experience in specialized geriatrics within the last 2 years – including assessment, development and implementation of care plans for adults with dementia, neurodegenerative conditions, mental health or addictions issues who exhibit responsive behaviours
  • Advanced clinical knowledge of dementia and the ability to appreciate the unique impact of the disease (i.e., amnesia, agnosia, aphasia, altered perception etc.) on the person and their support system
  • Demonstrated knowledge of home and community-based care practices, community and multi-disciplinary resources, and the ability to provide linkage to the appropriate services necessary to best serve the person
  • Demonstrated strong assessment skills and knowledge of medical challenges related to the aging process
  • Proven ability to liaise with Home and Community Care Services and other community agencies to promote effective communication and positive relations among care partners
  • Excellent interpersonal and team building skills that enable rapport building with stakeholders to achieve outcomes of a complex nature
  • Demonstrated leadership skills that foster engagement and strengths-based outcomes
  • Ability to practice independently, as well within an interprofessional team  
  • Excellent organizational, interpersonal, problem solving, and decision-making skills
  • Strong organizational and critical thinking skills and ability to prioritize tasks
  • Excellent verbal communication and documentation skills
  • Ability to provide written consultations and co-develop plans of care for identified responsive behavioural presentations
  • Proven ability to prioritize and effectively manage a diverse caseload, using exceptional time management skills
  • Demonstrated commitment to continuing education

ASSETS

  • Experience working in both acute care and long-term care settings preferred
  • Experience with EPIC
  • Proficient in Microsoft Office Applications (Word, PowerPoint, Teams, Excel)
  • Gentle Persuasive Approach (GPA) Certified Instructor

EMPLOYMENT REQUIREMENTS

Successful applicants are required provide a satisfactory vulnerable sector criminal reference check, that was complete less than one year from the date of hire. This requirement must be met within the first sixty (60) days of employment.

Additionally, to the above, successful applicants are also required to complete the Health Care Worker Health review process which includes providing an up-to-date immunization record to ensure compliance with the Ontario Hospital Association Communicable Diseases Surveillance Protocols within the first thirty (30) days of employment. Northumberland Hills Hospital operates a mandatory COVID-19 vaccination policy for all staff.
