LOCATION
Located approximately 100 kilometres east of Toronto, NHH is a vibrant community hospital delivering a broad range of acute, post-acute, outpatient and diagnostic services. Directly serving a growing population of more than 60,000 residents spread across a mix of urban and rural settings, NHH directly employs approximately 750 people and relies on the additional support provided by a highly engaged and supportive network of physicians, midwives and volunteers.
ROLE OVERVIEW
Reporting to the Manager of Mental Health, and in combination with the Assertive Community Treatment Team (ACTT), the RN provides psychiatric nursing care, psychosocial rehabilitation and support to individuals with a serious and persistent mental illness in a community setting.
- Provides mental health nursing care to clients of the ACTT Team in collaboration with members of the multidisciplinary team
- Utilizing a broad theoretical foundation, assesses clients for their bio-psychosocial needs
- Collaborates with the interdisciplinary team, the client and support network in the development, implementation and evaluation of a comprehensive plan of care that includes rehabilitation and support of the client
- Provides crisis intervention
- Provides comprehensive support to clients in medication management, including assisting clients to organize their medications and administering medication on a daily and PRN basis as indicated
- Updates client files and records clients’ visits and interventions as per policy
- Identifies community resources and guides individuals and families in their use as appropriate
- Participates as a member of an interdisciplinary team in providing comprehensive mental health services
- Provides for professional growth of self and others. Broadens knowledge and improves skills through formal and continuing education programs
- Participates in clinical care planning conferences, intake and staff meetings
- Participates in community programs directed toward the promotion of mental health
- Follows safe and healthy work practices and maintains a safe and healthy work environment through attendance at educational programs; uses established reporting mechanisms for injuries, illness and hazards and adherence to health and safety related policies and procedures.
QUALIFICATIONS/REQUIREMENTS
- Current Registration with the College of Nurses of Ontario
- Three years of recent Mental Health Nursing experience preferably in a community setting (preferred)
- Demonstrates high level of skills to solve complex problems, negotiate and communicate with clients, other service providers and members of the community required
- Working knowledge of the Mental Health Reform, Mental Health Act, privacy legislation required
- Evidence of ongoing continuing education courses in Mental Health or related fields required
- Current BCLS
- Valid driver’s license and insurance required
- Successful completion of a criminal reference review required
ASSETS
- Bachelor of Science Degree in Nursing preferred
- Certificate in Mental Health Nursing an asset
EMPLOYMENT REQUIREMENTS
Successful applicants are required provide a satisfactory vulnerable sector criminal reference check, that was complete less than one year from the date of hire. This requirement must be met within the first sixty (60) days of employment.
Additionally, to the above, successful applicants are also required to complete the Health Care Worker Health review process which includes providing an up-to-date immunization record to ensure compliance with the Ontario Hospital Association Communicable Diseases Surveillance Protocols within the first thirty (30) days of employment. Northumberland Hills Hospital operates a mandatory COVID-19 vaccination policy for all staff.
Due to the ongoing pandemic, Northumberland Hills Hospital may choose to conduct parts/all the hiring process virtually. The successful incumbent should expect the role to be in person however a hybrid working arrangement may also be considered subject to operational needs. Furthermore, we are a scent-free environment.
