LOCATION

Located approximately 100 kilometres east of Toronto, NHH is a vibrant community hospital delivering a broad range of acute, post-acute, outpatient and diagnostic services. Directly serving a growing population of more than 60,000 residents spread across a mix of urban and rural settings, NHH directly employs approximately 750 people and relies on the additional support provided by a highly engaged and supportive network of physicians, midwives, and volunteers.

ROLE OVERVIEW

The Clinical Supervisor is a leader who facilitates and mentors the interprofessional team to effectively manage utilization of resources and promotes opportunities for patients to be supported in the right place at the right time to receive the right care. The Clinical Supervisor is responsible for leading and implementing bed utilization protocols to achieve efficient and timely access to care across the organization that align with the targeted patient access and flow goals. You will work closely with the interprofessional team to facilitate the development of individualized plans to facilitate a safe, appropriate, and timely discharge, including liaising with Home and Community Care partners to ensure practices align with the Home First Philosophy. You will counsel patients and families related to hospital admission and discharge to provide continuity of care and support their individualized care goals. In addition, you will also act as a coach and mentor for staff to empower critical thinking to assist in challenging circumstances, from the lens of access and patient flow across the organization and system.  

RESPONSIBILITIES

  • Facilitating daily access and flow to optimize patient throughput, using available tools/dashboards and through engagement of the interprofessional care team across the organization.
  • Maintain a tracking system for all patients and their respective care pathways.
  • Support the implementation or the organization’s strategic plans and participate in continuous evaluation and monitoring of clinical activities to ensure strategic goals are met.
  • Facilitate interprofessional discussions with patients and families to achieve shared goals of care and disposition planning
  • Develop and implement policies/guidelines to support best practices in patient pathways and other corporate initiatives across the organization.
  • Maintain and strengthen collaborative relationships with community partners. Represent the organization to further collaborate with community partners on various projects and initiatives as assigned.
  • Maintain a high level of clinical competence, participate in professional/program development, clinical training opportunities and staff training, and remain current in the knowledge of and adhere to agency policies and procedures as well as relevant legislation.
  • Collect data and analyze against productivity and quality benchmarks and efficiencies.
  • Identify organizational risks and assist the interprofessional team to problem solve and support solutions to mitigate risk.    
  • Utilize shared governance model to participate, develop, and implement quality improvement initiatives.
  • Lead or assist with the integration of client satisfaction initiatives; ensure that all referred client concerns and complaints are satisfactorily addressed and documented in a timely manner.

In this role you will have a well-developed understanding of community supports and build collaborative relationships with community partners to facilitate and inform future service opportunities for patients transitioning from the hospital to the community.

The successful candidate will demonstrate exceptional leadership and case management skills, as well as possess a level of understanding of care across the continuum. In addition, they will demonstrate a high level of professional integrity and accountability and play a key role in system development and assume responsibilities as part of committees and working groups as appropriate.

QUALIFICATIONS/REQUIREMENTS

  • Baccalaureate degree in a health-related field
  • Registration in good standing with a relevant professional college
  • Knowledge of relevant legislation and a comprehensive knowledge of healthcare and community resources required, including the Home First Philosophy
  • Minimum of 5 years of current experience in an acute care setting with coordination of community services preferred
  • Experience in Patient Flow, Discharge Planning and Coordination in an interdisciplinary health care setting preferred
  • Experience in project management

  • Shows initiative and leadership in all duties and responsibilities of the position.
  • Knowledge and proficiency in the use of electronic clinical information systems (EPIC preferred) and office support software is required.

EMPLOYMENT REQUIREMENTS

Successful applicants are required provide a satisfactory vulnerable sector criminal reference check, that was complete less than one year from the date of hire. This requirement must be met within the first sixty (60) days of employment.

Additionally, to the above, successful applicants are also required to complete the Health Care Worker Health review process which includes providing an up-to-date immunization record to ensure compliance with the Ontario Hospital Association Communicable Diseases Surveillance Protocols within the first thirty (30) days of employment. Northumberland Hills Hospital operates a mandatory COVID-19 vaccination policy for all staff.

Due to the ongoing pandemic, Northumberland Hills Hospital may choose to conduct parts/all the hiring process virtually. The successful incumbent should expect the role to be in person however a hybrid working arrangement may also be considered subject to operational needs.
