LOCATION AND ABOUT NHH
Located approximately 100 kilometres east of Toronto, NHH delivers a broad range of acute, post-acute, outpatient and diagnostic services. Acute services include emergency and intensive care, medical/surgical care, obstetrical care and palliative care. Post-acute specialty services (PASS) include restorative care and rehabilitation. ROLE OVERVIEW
The Speech-Language Pathologist in the Stroke Recovery Clinic provides consultation, assessment, intervention and education regarding disorders of communication and dysphagia related to stroke in accordance with the guidelines of the College of Audiologists and Speech-Language Pathologists of Ontario, and the hospital. Currently vacant, 0800-1600, one shift a week.
Primary Responsibilities:
- Receive and acknowledge stroke rehab referrals from providers for assessment and treatment
- Receive consults from other health care professionals and obtain physician referral as necessary
- Review medical history, interpret evaluations and test results pertinent to speech, language, voice and swallowing disorders
- Conduct speech, language and dysphagia assessment as indicated
- Identify speech, language, voice and swallowing disorders
- Relate findings to patient/family/physician and other team members
- Develop appropriate patient/family centred treatment goals
- Provide counselling, education and training to patients and family regarding communication disorders and dysphagia
- Implement appropriate treatment plan for identified disorders
- Communicate/demonstrate and involve other team members in treatment plan interventions to be carried out
- Monitor and review the effectiveness of the treatment plan and modify as necessary
- Complete a discharge note and recommend follow-up interventions/services when indicated
- Facilitate continuity of patient care by liaising with other caregivers as appropriate (e.g. pre-admission/post-discharge services)
- Develop and participate in in-service education
- Attend and contribute to medical rounds, conferences and lectures
- Attend seminars, conferences and courses to maintain professional and technical
- Participate in clinic goal setting, evaluation and long-term planning
- Comply with hospital guidelines - e.g. confidentiality, infection control, WHMIS, code procedures, etc.
- Monitor operation of department equipment and reports malfunctions to director
- Comply with hospital guidelines - e.g. confidentiality, infection control, WHMIS, code procedures, etc.
- May be required to support inpatient allied health team related to HHR and hospital demands
QUALIFICATIONS/REQUIREMENTS
- Master's degree in Speech Language Pathology
- Certification with the College of Audiologists and Speech-Language Pathologists of Ontario
- Dysphagia experience
- Experience working with stroke patients in an in-patient or out-patient setting preferred
- Recent relevant experience with Adult population
- Good decision making and deductive reasoning skills
- Excellent verbal and written communication skills
- Excellent interpersonal skills and ability to work effectively with patients/families, other professionals and interdisciplinary team members
- Excellent organizational and time management skills
- Professionalism, independence and dedication to client-centred practice
PHYSICAL EFFORT AND WORKING CONDITIONS
- This position is staffed casual outpatient stroke clinic
- There may be exposure to bodily fluids and infectious diseases when working with patients

EMPLOYMENT REQUIREMENTS
Successful applicants are required provide a satisfactory vulnerable sector criminal reference check, that was complete less than one year from the date of hire. This requirement must be met within the first sixty (60) days of employment.
Additionally, to the above, successful applicants are also required to complete the Health Care Worker Health review process which includes providing an up-to-date immunization record to ensure compliance with the Ontario Hospital Association Communicable Diseases Surveillance Protocols within the first thirty (30) days of employment. Northumberland Hills Hospital operates a mandatory COVID-19 vaccination policy for all staff.
