LOCATION AND ABOUT NHH

Located approximately 100 kilometres east of Toronto, NHH delivers a broad range of acute, post-acute, outpatient and diagnostic services. Acute services include emergency and intensive care, medical/surgical care, obstetrical care and palliative care. Post-acute specialty services (PASS) include restorative care and rehabilitation. Mental health care, cancer and supportive care, dialysis and other ambulatory care clinics are offered on an outpatient basis through partnerships with regional centres and nearby specialists. NHH offers a full range of diagnostic services, including magnetic resonance imaging (MRI), computed tomography (CT) and mammography. The hospital serves the catchment area of west Northumberland County. A mixed urban and rural population of approximately 67,000 residents, west Northumberland comprises the Town of Cobourg, the Municipality of Port Hope, Alderville First Nation and the townships of Hamilton, Cramahe and Alnwick/Haldimand. ROLE OVERVIEW

The successful applicant will provide high quality patient care in accordance with the Professional standards of the College of Nurses of Ontario and in alignment with best practices and guidelines established by the Regional Cancer Program and Cancer Care Ontario. The individual, as a member of the Cancer and Supportive Care program, has a significant role in promoting health, preventing illness, and supporting patients and families along the healthcare continuum, using a patient centered care approach. The successful applicant will contribute to a positive workplace culture, working in collaboration with an interdisciplinary team, while adhering to principles of patient safety and demonstrating high regard for quality improvement and continuous learning. Primary responsibilities include:

  • Assessing the physical, psychosocial, spiritual, cultural, socioeconomic, and educational needs of the patient and family
  • Anticipating and identifying crisis situations arising from specific illness and or illness management in both the patient and family
  • Completion of symptom management screening tools & telephone triage as required
  • Developing, implementing, assessing, evaluating, and revising individualized plans in collaboration with the patient (and family) and the health care team
  • Continuously monitoring patient (and family’s) response to illness and treatments and respond accordingly
  • Initiating and coordinating appropriate referrals to other health care professionals based on assessment and referral criteria as required
  • Working autonomously and collaboratively with all members of the interprofessional team as the situation warrants

QUALIFICATIONS/REQUIREMENTS

  • Currently registered (in good standing) with the College of Nurses of Ontario
  • Current B.C.L.S.
  • Recent experience in chemotherapy, hematology, cancer care and supportive care is preferred
  • Completed DeSouza Foundations in Oncology nursing practice and Provincial Standardized Chemotherapy and Biotherapy Course or equivalent prior to starting the position is required
  • Completion of the De Souza foundations in oncology nursing practice or equivalent prior to starting independently in the position required
  • Commitment to complete the Provincial Standardized Chemotherapy and Biotherapy Course at next offering is required
  • Oncology Nursing Certificate from an accredited college – completed or currently enrolled is preferred
  • Demonstrated competence in CVAD care and maintenance and I.V. Therapy
  • Added nursing skills and demonstrated competency, including but not limited to- aseptic techniques, wound management, IV initiation and maintenance, venipuncture, Phlebotomy, Biopsy recovery, Blood product administration, and health assessment
  • Demonstrated commitment to continuing education
  • 2-3 years’ experience in acute care nursing within the last 5 years is preferred; wound care experience an asset
  • Demonstrates excellent written & oral communication skills, critical thinking, time management & problem-solving skills
  • Demonstrates knowledge of EPIC, computer systems and software
  • Applicant has a demonstrated understanding of Northumberland Hills Hospital’s Mission, Vision and Values and the CARE pillars of the strategic plan
  • Demonstrates strong interpersonal skills and the ability to foster therapeutic relationships, while working through emotionally intense environments with vulnerable patients and families
  • Proven to have a satisfactory attendance and performance record, with capacity to maintain these standards

EMPLOYMENT REQUIREMENTS

Successful applicants are required provide a satisfactory vulnerable sector criminal reference check, that was complete less than one year from the date of hire. This requirement must be met within the first sixty (60) days of employment.

Additionally, to the above, successful applicants are also required to complete the Health Care Worker Health review process which includes providing an up-to-date immunization record to ensure compliance with the Ontario Hospital Association Communicable Diseases Surveillance Protocols within the first thirty (30) days of employment. Northumberland Hills Hospital operates a mandatory COVID-19 vaccination policy for all staff.
