Northumberland PATH

This exciting local project was launched in June 2012 with a $3-million research grant from The Change Foundation to improve seniors' experiences as they move in, out of, and across Ontario's health system. Led by NHH, the pilot project involved local seniors, caregivers and close to a dozen health and community agencies. The history of the project, and some of the many lessons learned, have been summarized by The Change Foundation in a three-part report, Charting a New PATH, published March 30, 2016, and available on www.changefoundation.ca.

While the initial pilot has now concluded, the work of PATH is still very much alive in our community. For the latest update on PATH's new project, supported by technology partner QoC Health and the Ontario Centres of Excellence, click here
