Reporting to the Director, the Pharmacist is responsible for the distributive and the clinical functions related to the practice of pharmacy throughout the organization.  

PRIMARY RESPONSIBILITIES:

  • Acts as a resource to the members of the interdisciplinary team.
  • Assists with the maintenance of the Formulary.

  • Monitors quality measures as assigned.

  • Assists with the development of policies and procedures related to drug utilization, distribution and control and drug interactions.

  • Assists in the investigation and follow up of drug interactions and medication errors.

  • Provides education to the interdisciplinary team members and patients.

  • Acts as a resource to the Manager of Pharmacy and other members of the interdisciplinary team in the provision of drug information as it relates to the administration of medications within NHH.

  • Assists the Manager in ensuring that the acts and regulations related to pharmacy practice are adhered to and recommends changes in the pharmacy practice area as appropriate.

  • Participates in the interdisciplinary team as assigned at patient care rounds and the respective team.

  • Maintains records as required by the Hospital, the Food and Drugs Act, The Ontario College of Pharmacists, Narcotics and Controlled Substances Act.

  • Ensures that all medications are distributed and administered in a safe and timely manner.

  • Provision of on-call support to interdisciplinary team is required.

QUALIFICATIONS REQUIRED:

  • Current registration in Part 1 of the Ontario College of Pharmacists Register.
  • Completion of a residency program in a hospital or equivalent clinical experience.

  • Oncology/CIVA Pharmacy Services experience preferred

  • Current membership with Canadian Society of Hospital Pharmacists preferred.

  • Strong clinical pharmacy skills to support NHH’s pharmaceutical care and medication reconciliation processes

  • Preference will be given to candidates with 2-3 year’s experience.
